MCQ and Report slides

Problem:
hi all, I am creating MCQ slides and I need to get score at the end of it. However, despite setting correct answers and scores for each question, the Report Slide is not calculating the score. You click all right answers it is still showing score as zero and result as failed! I tried creating a new file altogether but still the issue persisted. What could be the error?
