Maybe I didn’t find the form myself, it’s hard to believe that this software doesn’t have form generation function.

Unfortunately, ActivePresenter doesn’t support form generation function.
You can consider inserting a grid box in a slide and use Preset Layouts to create the layout/ the form that you want.
